Existing Patients of Dr Quinn
If you have already seen Dr Quinn at Village Medicine, we invite you to book online.
If you are an existing patient of Dr Quinn and are booking at Village Medicine for the first time, we kindly ask that you:
- Complete the 'Transfer of Records' form before your first appointment. Your completed form can be returned via email at hello@villagemedicine.com.au or fax.
- Call the practice on 02 5643 9277 to set up a profile so you can book online.
Cancellations
We kindly request that patients provide at least 24 hours notice if they need to cancel or reschedule an appointment. Late cancellations or non-attendance will incur the full fee, reflective of the appointment type booked.
