If you are an existing patient of Dr Quinn and are booking at Village Medicine for the first time, we kindly ask that you:
- Complete the 'Transfer of Records' form before your first appointment. Your completed form can be returned via email at hello@villagemedicine.com.au or fax.
- Call the practice on 02 5643 9277 to set up a profile so you can book online.
Cancellations
We kindly request that patients provide adequate notice if they need to cancel or reschedule an appointment. Late cancellations or non-attendance will incur a fee, reflective of the appointment type booked.
